Bowtie, Upbeat and Hemi-seesaw Nystagmus in Medial Medullary Infarction

چکیده
Medial medullary infarction is characterized by contralateral paralysis of the arm and leg, contralateral loss of position and vibration sense, and an ipsilateral paralysis of the tongue. Rarely, upbeat nystagmus has been described. Bowtie nystagmus, which refers to oblique upbeat nystagmus with horizontal quick phases that alternate from right to left, has been reported in association with tobacco inhalation, c e r e b e l l a r d i s e a s e , 2 and brainstem anormalies. However, the occurrence of upbeat and bow-tie nystagmus and their transition into hemi-seesaw nystagmus has not been reported in medial medullary infarction.

Upbeat Nystagmus: Clinicoanatomical Correlations in 15 Patients
BACKGROUND AND PURPOSE The mechanism of upbeat nystagmus is unknown and clinicoanatomical correlative studies in series of patients with upbeat nystagmus are limited. METHODS Fifteen patients with upbeat nystagmus received full neuro-ophthalmological evaluation by the senior author. Nystagmus was observed using video Frenzel goggles and recorded with video-oculography. Brain lesions were docu...

Medial medullary infarction is usually manifested as hypoglossal palsy, limb weakness, impairment of proprioception, and oculomotor disturbance. We report a case with the unusual presentation of sensory ataxia. A 71 year-old male presented with ataxia and disequilibrium. Bilateral dysmetria, truncal ataxia, Upbeat nystagmus, and impaired vibration and position sense were the clinical features. ...
متن کاملDiagnostic confusion resolved by being upbeat
The classical stroke presentation - captured by the public health campaign mnemonic FAST (face, arm, speech, time) - does not apply in a large number of stroke cases; yet establishing a prompt diagnosis is imperative for optimal management. Here, we describe a patient with acute bulbar weakness, numbness in all extremities and an apparently normal magnetic resonance imaging (MRI) of the brain u...
متن کاملUpbeat nystagmus in a patient with a small medullary infarct.
A 43 year old man presented with decreased sensitivity in the left side of the face and both upper limbs, and with upbeat nystagmus and skew deviation. MRI demonstrated a well defined lesion compatible with an infarct in the left side of the medulla, caudal and ventral to the vestibular nuclei, possibly involving the most caudal of the perihypoglossal nuclei, the nucleus intercalatus. Ocular mo...
متن کاملPrimary Position Upbeat Nystagmus during an Acute Attack of Multiple Sclerosis
BACKGROUND AND PURPOSE Ocular manifestation is one of the frequent signs of an acute attack in multiple sclerosis (MS), although primary position upbeat nystagmus (PPUN) is rare. The purpose of this study is to determine the incidence of PPUN in MS and to determine the lesions that are responsible for this sign. METHODS The medical records of 120 MS patients with acute brain lesions were revi...
